Successful Life Project Nurse - Registered Nurse Senior|
As a member of the Successful Life Project (SLP), the Successful Life Project Nurse functions as an integral member of the Successful Life Project providing advanced assessment and clinical consultation of health care services for Therapeutic Follow-up Members. The Jensen Settlement Agreement Comprehensive Plan of Action requires therapeutic follow-up of Jensen Class Members and people previously served at MSHS-Cambridge (collectively referred to as Therapeutic Follow-up Members).
Job Responsibilities Include:
•Review the Therapeutic Follow-up Member’s health care status, including medication and treatment plans, to determine if health service delivery meets acceptable standards of care. Make recommendations that improve the delivery of health services and increase the stability of the person’s community living.
•Work directly with person(s) at-risk and their team, carrying out health promotion programs and training at all levels of prevention, basic data collection and analysis, program planning, outreach activities and programmatic support.
•Maintain a supportive and evaluative function to the program serving the Therapeutic Follow-up Member(s), to ensure that the program effectively implements recommendations.
•Functions as a generalist with expertise in public health nursing and mental health nursing, and with consultation as needed from colleagues with complementary expertise.
•Assist with assigned compliance projects that impact other programs across the Department of Human Services (“DHS”).
•Responsible for conducting research, and reviewing state/federal regulations and other best practices to help establish program goals and standards and assist leadership with ongoing quality assurance initiatives and their effectiveness.
Qualifications & Requirements
Current licensure in Minnesota as a Registered Nurse AND
•Bachelors of Nursing Degree plus two (2) years Registered Nurse experience OR
•Associate’s Degree or Diploma in Nursing plus three (3) years Registered Nurse experience.
Certification in area of Expertise = 1 year of Registered Nurse experience
